The best advice would be little and often. What is often considered the best option outside of a drip system is around 5% a day. Obviously nobody has the time to do this!!

I do 10% 3 times a week. I think you will find most peoples water change schedules are different

Its good practice to use a water conditioner if your water contains chlorine and Chloramines. which most peoples do, to make it drinkable for us. But other than that water from the tap yes, as close to the water parameters of the tank as possible. And try and keep it the same temp ish. So not to shock them. You must actually remove water too and not just let it evaporate. This removes nitrates, the only way to get rid of them!

Very good point on not just letting it evaporate before you add water....Many people do that and it will screw up the concentration of solutes!~
