How do you think its adding more ammonia by water changes? What kind of filtration, size of tank, how many fish?

So you claim that your tap water has a NH4 reading of 8+ ppm (mg/l).
That is quite impossible.
Sometimes waterworks release too much chlorine or chloramine (chlorine+ammonia) and the levels may be too high for a short time. You can find it out by just asking about it from your waterworks.
